Membership Application Form

The completion of the Membership Application Form is required for all candidates before joining the church. The application form is designed to allow us to get important information about you so that we may best serve you and encourage you in God's Kingdom work.

The form is provided to all membership candidates at the beginning of the Membership Class. However, you may also download the Membership Application Form here (as a PDF file) and complete it prior to the class. If you elect to download this form, please bring the completed form with you to class.
